I hereby acknowledge, agree, and confirm my understanding of the activities offered at the Muslim American Society. I certify that I am the parent or legal guardian of the minor participant and have the legal authority to sign this agreement on their behalf. I affirm that both myself and my child(ren) are adequately qualified, in good health, and in proper physical condition to participate in these activities. I willingly ACCEPT ALL RISKS associated with these activities. Accordingly, I release, discharge, and agree not to sue, bring a claim against, or hold liable the Muslim American Society, including its administrators, directors, agents, officers, members, volunteers, employees, other participants, sponsors, advertisers, and, if applicable, owners and lessors of the premises where the activity occurs. This release encompasses all liability, claims, demands, losses, or damages that may arise due to negligence on the part of the mentioned parties or otherwise, including negligent rescue operations. Additionally, I agree that if, despite this release and waiver of liability, a claim is made on my behalf against any of the aforementioned parties, I will indemnify, defend, and hold harmless each of them from any expenses, attorney fees, losses, liabilities, damages, or costs incurred as a result of such a claim. I further understand that photographs and/or video recordings may be taken during programs and events. I grant permission to the Muslim American Society to use images or recordings of myself and/or my child(ren) for educational, promotional, and media purposes, including but not limited to social media, websites, print materials, and internal publications, without compensation, and I waive any right to inspect or approve the finished materials.
