I acknowledge that participation in basketball involves physical activity and risk of injury. I voluntarily give permission for my child to participate and agree to release and hold harmless California Islamic Center (CIC), its staff, coaches, volunteers, and organizers from any liability, claims, or damages arising from participation.

I understand that there is a $30 camp fee per player. I acknowledge that no refunds will be given for no-shows or cancellations. All proceeds go directly to CIC Youth. I understand that after completing this form, I will be added to a WhatsApp group, where additional camp information will be shared.
